Sometimes it can be difficult to take the first step to go back to the dentist, or to find a new dentist that the whole family can see. We will take care of you and your family, and will make your visit as pain free and stress free as possible.

Also for your convenience, you can print off our new patient information form prior to your appointment. There is a front and back to the form, please fill out both. 

Also if you have insurance, please bring your card with you. If your card has another guarantor other than yourself, such as a parent or spouse, please bring their social security number with you, so we can run your benefits while you are in the office.
